Zusammenfassung

Die neuralgische Amyotrophie ist eine Erkrankung des peripheren Nervensystems, die sich durch starke neuropathische Schmerzen, gefolgt von peripheren Paresen äußert. Es wird zwischen einer hereditären und einer idiopathischen Form differenziert, bei der eine autoimmunologische Genese angenommen wird. Konservativ wird die Erkrankung mit nichtsteroidalen Antirheumatika (NSAR), Opioiden und Glukokortikoiden behandelt. Trotz Therapie persistieren in über 50 % der Fälle Beschwerden in Form von Schmerzen oder Lähmungen. Durch Inflammation kann es zu Strikturen und Torsionen peripherer Nerven kommen, die sich bildgebend mittels Nervensonographie sowie Magnetresonanz(MR)-Neurographie und später intraoperativ nachweisen lassen. Anhand der aktuell zur Verfügung stehenden Daten können solche Patienten von einer neurochirurgischen Behandlung profitieren.
