Zusammenfassung

Die Gefäßbeurteilung mittels Duplexsonografie setzt genaue Kenntnisse der normalen Morphologie und Hämodynamik, aber auch der Gefäßpathologien sowie eine klinische Untersuchung voraus. Es werden die normale Morphologie und Hämodynamik und der Untersuchungsablauf beschrieben sowie die Befunde von arterieller Verschlusskrankheit und Venenthrombose, aber auch seltenerer Gefäßerkrankungen dargestellt. Die farbkodierte Duplexsonografie von Arthropathien wird am Beispiel der rheumatoiden Arthritis behandelt. Geräteeinstellung, Untersuchungstechnik und die Validität von Doppler-Sonografie und Scoring-Systemen sowohl bei Synovitiden wie auch bei Tenosynovitiden werden erörtert. Die Bedeutung des Dopplers im klinischen Alltag als auch dessen Einsatzgebiete bei anderen rheumatischen Erkrankungen (Osteoarthrose, Spondyloarthritiden, Kristallarthropathien) werden diskutiert. Zudem werden Indikationen für die Farb-Doppler-Untersuchung periartikulärer Erkrankungen (u. a. Raumforderungen) aufgezeigt.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
