24. Endovenöse thermische Ablationsverfahren

Zusammenfassung

Die Prinzipien der klassischen Varizenchirurgie (KV) gehen zurück auf die Konzepte zur Beseitigung des Refluxes im epifaszialen Venensystem, wie sie im 19. Jahrhundert von Trendelenburg (1891) u. a. entwickelt und wie sie in unserer Zeit unter dem Terminus Ausschaltung des Rezirkulationskreises bei der primären Varikose in Sinne von Hach (1994) weiterentwickelt und perfektioniert wurden. Die KV gilt als relativ invasives Verfahren. Das Streben nach Minimierung der Invasivität führte zur Entwicklung der endovenös thermischen Verfahren. Diese schalten analog der Operation die Refluxstrecke aus, verzichten aus Gründen der Invasivität auf die operative Crossektomie und unterscheiden sich dadurch von den Prinzipien der klassischen Varizenchirurgie. Die Radiofrequenzablation (RFA) wurde 1998 zur Behandlung in Deutschland zugelassen, die endovenöse Lasertherapie (ELT) 1999. Sie haben sich als alternative Verfahren zur offenen Operation mittlerweile etabliert. Entsprechend der Empfehlung der aktuellen Leitlinie sollten sie den Patienten als Alternative zu anderen Therapieverfahren angeboten werden.

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
