5. Radiologische Diagnostik von pulmonalen Metastasen

Zusammenfassung

Die radiologische Bildgebung in der Diagnostik von Lungenmetastasen umfasst zum einen die klassische Röntgenaufnahme und deren Weiterentwicklung in Form der digitalen Tomosynthese und zum anderen die Schnittbildgebungsverfahren CT und MRT. Dabei hat die Computertomographie aufgrund ihrer breiten Verfügbarkeit und ihrer hohen Sensitivität den höchsten Stellenwert. Eine zunehmend wichtige Rolle im onkologischen Staging, aber auch in der Differenzierung von pulmonalen Herden spielen Hybrid-Bildgebungsverfahren und hier in erster Linie die PET/CT mit unterschiedlichen Tracern. Der Stellenwert der modernsten Modalität PET/MRT in der Diagnostik von Lungenmetastasen kann gegenwärtig noch nicht bestimmt werden.

Bei schwerer Aortenstenose und obstruktiver KHK empfehlen die Leitlinien derzeit eine chirurgische Kombi-Behandlung aus Klappenersatz plus Bypass-OP. Diese Empfehlung wird allerdings jetzt durch eine aktuelle Studie infrage gestellt – mit überraschender Deutlichkeit.

S3-Leitlinie „Diagnostik und Therapie des Karpaltunnelsyndroms“

Das Karpaltunnelsyndrom ist die häufigste Kompressionsneuropathie peripherer Nerven. Obwohl die Anamnese mit dem nächtlichen Einschlafen der Hand (Brachialgia parästhetica nocturna) sehr typisch ist, ist eine klinisch-neurologische Untersuchung und Elektroneurografie in manchen Fällen auch eine Neurosonografie erforderlich. Im Anfangsstadium sind konservative Maßnahmen (Handgelenksschiene, Ergotherapie) empfehlenswert. Bei nicht Ansprechen der konservativen Therapie oder Auftreten von neurologischen Ausfällen ist eine Dekompression des N. medianus am Karpaltunnel indiziert.
