Mind the gap: understanding the discordance between the ATS and ESICM ARDS guidelines—the ATS perspective

In 2017, the American Thoracic Society (ATS), the European Society of Intensive Care Medicine (ESICM), and the Society of Critical Care Medicine (SCCM) published a collaborative guideline addressing patients with acute respiratory distress syndrome (ARDS) summarizing the existing evidence and providing recommendations for high-priority interventions [

2]. With several new randomized controlled trials (RCTs) and following the influx of ARDS related to coronavirus disease 2019 (COVID-19), a decision was made to update these guidelines based on the latest evidence. Rather than the previous multi-societal approach, ATS and ESICM undertook independent efforts [

4]. While there are some advantages to this dual approach, it also risks leaving clinicians grappling with the differences in recommendations between the two publications. …
