Laparoscopic Right Colectomy for Cancer: Should We Ligate the Vessel Intracorporeally or Extracorporeally?

We have read with great interest the paper by Garoufalia et al. [
1], that aimed at comparing intracorporeal and extracorporeal ligation of the vascular pedicle during laparoscopic right colectomy for cancer. They evaluated intraoperative short-term outcomes and pathology. A total of 271 patients were included in this retrospective comparative study: vessel ligation was performed intracorporeally in 171 (63%) patients, while it was done extracorporeally in the remaining 100 (37%) patients. Mean operative time of laparoscopic right colectomy with intracorporeal vessel ligation was 30 min longer. Postoperatively, no significant differences were observed in terms of morbidity: in particular, the rates of prolonged postoperative ileus, bleeding, surgical site infection, and need for reoperation were similar. After matching the two groups of patients for the extension of the surgical resection, the median number of lymph nodes harvested was significantly higher (28.5 vs. 24) in the group of patients who received intracorporeal vessel ligation, while the median number of positive lymph nodes was similar. Also, the median length of the resected specimen differed significantly: 26 in the intracorporeal and 23 cm in the extracorporeal. …
